Care, Planning & Guardianship Services

Guardianship of the Person
Court-authorized support for healthcare and daily living decisions.
This service assists with personal, medical, and care-related decisions when an adult is unable to make them safely and independently.
Guardianship of the person is carried out under court authority and ongoing oversight, with an emphasis on supported decision-making—helping individuals participate in choices to the greatest extent possible.
We take a hands-on, relationship-based approach. We get to know the person, maintain regular in-person visits, and show up for medical appointments and care meetings when needed.
All work is guided by the individual’s wishes, safety, and well-being, using the least restrictive approach to support independence, dignity, and quality of life.

Elder Care Coordination
Elder Care Coordination provides practical, ongoing support for older adults and families as needs change.
We help coordinate services, communicate with providers, and support safe aging at home—without taking over decision-making or involving the court unless truly needed.
Support may include help with technology troubleshooting, setting up or correcting direct deposit, organizing bills and paperwork, arranging home safety updates (such as grab bars or accessibility supports), and coordinating cleaning, organization, or downsizing services.
This service is especially helpful when care involves multiple providers, family members live at a distance, or situations feel complicated but do not yet require conservatorship or guardianship.

Planning conversations may include discussing advance care planning and healthcare preferences in plain language, identifying trusted people to support decision-making, exploring supported decision-making and alternatives to court involvement, and organizing important information so it is accessible when needed. The goal is clarity and confidence, not predicting every outcome.

You should'nt have to be on a very long waiting list
During that time, life doesn’t pause. Needs change; health can decline; finances can become unstable; and family caregivers can burn out. Small issues quietly turn into crises.
We work as a partnership, not a hierarchy. Unlike large, layered guardianship offices, our small, community-based practice stays present, personal, and accountable 24/7.
